How we get from a category thesis to a national survivorship primary care network. Each phase includes the milestones that trigger the next one — no filler, no vanity metrics.

1. Phase 0

   Thesis & clinical foundation

   Complete
   • Category thesis: cancer survivorship primary care is under-built despite 18M+ US survivors.
   • Clinical Advisory Board seats defined across adult oncology, pediatric LTFU, cardio-onc, endocrine, primary care, behavioral health, pharmacy, equity, and survivor voice.
   • Protocol v1 aligned to NCCN Survivorship, ASCO, and COG-LTFU 6.0.

2. Phase 1

   Flagship clinic & platform

   In progress
   • Flagship survivorship primary care clinic operating end-to-end (intake through care-plan handoff).
   • Spark OS platform live for care plans, surveillance schedules, deprescribing, and de-identified cohort insights.
   • Employer + payer LOIs signed in flagship region.

3. Phase 2

   Franchise launch

   Next
   • FDD registered; first 10 franchisee cohort selected from qualified oncology, primary care, and multi-unit operators.
   • Franchisee playbook: real-estate, staffing, payer credentialing, and 90-day panel-ramp plan.
   • National training academy for MD/APP, RN navigator, MA, and clinic manager roles.

4. Phase 3

   Payer + employer scale

   Planned
   • Value-based contracts across commercial, Medicare Advantage, and self-insured employers.
   • Public outcomes dashboard updated quarterly, tied to protocol changelog.
   • AYA transition program live in every clinic with pediatric partner referral network.

5. Phase 4

   National network

   Long-range
   • Life Spark clinic in every US metro > 500K and every state.
   • Survivorship registry contributing de-identified data to peer-reviewed research.
   • Category leadership: Life Spark is the default answer to "who does survivorship?"
