Clinical Advisory Board

Every clinical decision in the Life Spark protocol — surveillance cadence, deprescribing criteria, transition milestones, vaccine timing — is governed by a multidisciplinary board with a voting survivor and caregiver seat. This page describes the seats, the charter, and the operating cadence. Named members are published on the protocol changelog when each version is released.

Board seats & charters

Adult survivorship oncology

Owns adherence of the adult protocol to NCCN Survivorship, ASCO survivorship guidelines, and disease-specific late-effect pathways.

Seat profile

Board-certified medical oncologist with a dedicated survivorship clinic and prior guideline-writing experience.

Pediatric / AYA long-term follow-up

Owns pediatric and AYA transition, COG-LTFU 6.0 alignment, and the childhood-cancer late-effects surveillance schedule.

Seat profile

Pediatric hematologist/oncologist directing a long-term follow-up program with COG affiliation.

Cardio-oncology

Owns anthracycline / HER2 / radiation cardiotoxicity surveillance, imaging cadence, and cardioprotective referral criteria.

Seat profile

Cardiologist with cardio-oncology fellowship or ACC Cardio-Oncology Section membership.

Endocrine late effects

Owns thyroid, adrenal, gonadal, growth, and metabolic surveillance for pediatric-onset survivors carried into adulthood.

Endocrinologist practicing in a survivorship or late-effects clinic.

Primary care integration

Owns the primary-care handoff, panel management, and quality-metric alignment (HEDIS, MIPS, value-based contracts).

Internal medicine or family medicine physician with population-health leadership experience.

Behavioral health & neurocognition

Owns distress screening thresholds, cognitive late-effect pathways, and integrated behavioral-health referral standards.

Psycho-oncology or neuropsychology clinician with survivorship experience.

Nursing & care coordination

Owns care-plan handoff, nurse navigator scripts, and the between-visit surveillance workflow.

Pharmacy & deprescribing

Owns the deprescribing worksheet, long-term therapy interactions, and vaccine/immunosuppression pharmacology.

BCOP-certified oncology pharmacist embedded in survivorship or ambulatory care.

Health equity & community

Owns access review, language access, cost-of-care transparency, and community-partner integration.

Public-health or community-medicine leader with cancer-equity portfolio.

Survivor & caregiver voice (2 seats)

Voting seats. Review every patient-facing artifact, plain-language reading level, and consent flow before release.

Operating cadence

Quarterly

Full board review of protocol changes, safety signals, and outcomes dashboard.

Monthly

Working-group calls per specialty seat with the medical director.

Governance principles

Serve on the board

We recruit board members with published survivorship work and lived expertise. Survivor and caregiver seats are compensated and voting. If you fit a seat above, tell us which one.
