Manager guidance

Comms templates

Announcement email, intranet blurb, and open-enrollment slide for launching the benefit — editable, brand-neutral, plain language.

Caregiver policy checklist

Nine-item review of caregiver leave, flexible scheduling, and bereavement policies — the levers that matter most for survivor households.

Drop-in RFP language

Copy any of the clauses below directly into a benefits RFP. They align with published guidelines and payer quality frameworks, and hold vendors accountable to survivorship-specific outcomes.

  1. RFP 01Vendor shall provide a written survivorship care plan within 90 days of enrollment for every member, based on NCCN Survivorship, ASCO survivorship, and (for pediatric-onset) COG-LTFU 6.0.
  2. RFP 02Vendor shall track and report, quarterly and de-identified: care-plan completion rate, on-schedule cancer screening, cardio-oncology surveillance adherence, distress screening completion, and avoidable ED encounters.
  3. RFP 03Vendor shall accept referrals from any oncologist without requiring transfer of primary oncology care, and shall not bill members for care-coordination time.
  4. RFP 04Vendor shall provide members a plain language treatment summary, medication list, and printable appointment prep before every visit.
  5. RFP 05Vendor shall accept a value-based arrangement structured as shared savings against a claims baseline agreed at contract start.
