For referring oncologists
Hand off survivorship. Keep the patient.
Life Spark is a survivorship home — not an oncology practice. We do not treat cancer, we do not compete with your practice, and we send every note back to you. If disease returns, the survivor comes straight back through your door.
The referring-oncologist promise
You keep the patient
We are the survivorship home — not oncology. If disease returns, we return them to you the same day with a full workup.
Closed-loop notes
Every visit note routes back to the referring oncologist and the primary care team via FHIR or fax. You never wonder what we did.
Guideline-anchored surveillance
COG-LTFU, NCCN Survivorship, ASCO Survivorship, and ASCO Cardio-Oncology. No experimental protocols. No cash-pay supplement pushing.
We do the coordination you don't have time for
Cardio-oncology, fertility, endocrine, cognition, psychosocial, financial navigation — held together, so your visit stays focused on the cancer.
Referral pathways
Who to send, when to send, what we do.
| Referring service | When to refer | What we deliver |
|---|---|---|
| Pediatric oncology → adult transition | Ages 16–21, or any survivor aging out of the children's hospital | Full transition-plan build, COG-LTFU surveillance schedule, adult specialist network, warm hand-off to Life Spark clinical team |
| Adult medical oncology → post-treatment | 6–12 months after last curative-intent treatment, or at your discretion | Portable survivorship chart, NCCN-aligned surveillance plan, cardio-oncology if anthracycline / chest-XRT / HER2 exposure, second-cancer screening cadence |
| Radiation oncology → late-effect surveillance | After radiation for pediatric or AYA cancer, or any high-dose adult field | Field-mapped late-effect risk profile, imaging cadence, endocrine and cardiac baseline, patient-owned summary document |
| Surgical oncology → post-op survivorship | Post-curative surgery, especially breast, colorectal, GYN, thyroid, urologic | Surveillance imaging cadence, hormone/endocrine follow-up, psychosocial + lymphedema screening, return referral if recurrence |
Referral workflow
Fits your existing workflow. Adds no chart burden.
- Step 01
Send a referral
One page: treatment summary (or discharge notes), current medications, and the survivor's contact info. Fax, EMR direct-message, or secure upload — whatever your workflow supports.
- Step 02
We contact the survivor within 3 business days
First visit scheduled within 2 weeks. Telehealth or in-clinic at any licensed Life Spark territory.
- Step 03
We build the plan and send it back
Full survivorship care plan, patient-owned copy, and a one-page summary to you and to the primary care team.
- Step 04
You get updates for as long as you want them
Every subsequent visit note routes back to you by default. Opt out at any time — the survivor's chart still travels with them.
