Pay with your CME allowance

Why it qualifies

• Clinical decision support. Cascade-of-care surveillance library aligned to NCCN, ASCO, and COG-LTFU guidelines.
• Practice-based learning & improvement (PBLI). Chart-review workflows, deprescribing worksheets, and cardio-oncology monitoring templates count toward MOC Part IV / Improvement in Medical Practice for ABIM, ABFM, and ABP.
• Point-of-care learning credit. Time spent using integrated reference material at the point of care is claimable under AMA PRA Category 1 Credit™ self-directed learning for eligible boards.
• Documented usage summary. We provide a downloadable activity log with dates, topics reviewed, and time on task — the record most CME committees ask for.

Three ways to get reimbursed

1. 1. Direct-bill your institution. Group and Health-System licenses can be invoiced to your CFO or department administrator. Email connect@lifesparksurvivorship.com for a W-9 and NET-30 invoice.
2. 2. Personal card, then reimburse. Pay at checkout, then submit the itemized PDF receipt plus the activity summary through your CME portal (MedHub, MyEvals, Concur, or Workday).
3. 3. CME debit card. Some systems (Cleveland Clinic, Kaiser, MD Anderson, academic AMCs) issue prepaid CME cards on the Visa network. These work at checkout like any card.

What we send you

• Itemized receipt with provider Tax ID and description of service
• Annual activity summary (topics reviewed, time on task, dates)
• Attestation letter for CME committee submission
• W-9 for institutional purchasing on request
