Impact & KPIs
Who this is for, what we'll measure, and when.
We only claim impact we can measure. This page lists the audiences Life Spark Survivorship Institute serves, the annual KPIs we're accountable to, how each metric is gathered (with your permission), and the date we expect to hit it.
Who Life Spark is for
Adult cancer survivors (all primaries, 18+)
Problem
Fragmented follow-up, late-effect surveillance gaps, no portable record.
What success looks like
One survivorship chart, on-time surveillance, less anxiety between visits.
AYA survivors (15–39) transitioning from pediatric to adult care
Problem
Cliff between pediatric oncology and adult medicine — up to 60% lost to follow-up in year one.
What success looks like
Structured transition plan, ≥80% survivors retained in follow-up at 12 months post-transition.
Caregivers & family members
Problem
Invisible workload, no shared plan, no way to see what's due next.
What success looks like
Read-access to the survivor's plan (with consent), reduced caregiver-burden score.
CSPN-verified clinicians & survivorship programs
Problem
No cohort view of overdue echo / lipids / DXA / second-cancer screening.
What success looks like
Cohort dashboards, closed-loop escalations, measurable reduction in overdue surveillance.
Employers & payers sponsoring survivorship benefits
Problem
No line-of-sight into whether survivorship benefits actually change outcomes.
What success looks like
De-identified cohort reporting on engagement, PRO trends, and screening completion.
Annual KPIs
Reviewed each year. Reported publicly in aggregate. Everything below runs on data you've consented to share.
| KPI | Target | By when | How we measure | Source |
|---|---|---|---|---|
| Monthly active survivors (MAS) | ≥ 60% of registered survivors log at least one tool entry per month | 12 months from launch | Consent-gated `tool_use` events + monthly cohort roll-up. | Consented analytics |
| Care-plan completion | ≥ 70% of survivors have a complete survivorship care plan (diagnosis, treatment summary, surveillance calendar) within 90 days of signup | 12 months | Server-side completeness check on care-plan fields — no PHI reported, only completion %. | Clinician dashboard |
| On-time cardio-oncology surveillance | ≥ 85% of anthracycline / HER2 / chest-RT survivors have an echo within guideline window | 18 months | Overdue-flag counters from the clinician dashboard, aggregated monthly. | Clinician dashboard |
| AYA transition retention | ≥ 80% of AYA transition-plan users still engaged 12 months after transition | 24 months | Consented longitudinal cohort; retention = ≥1 tool entry in the 12th month. | Consented analytics |
| Distress reduction | Median NCCN Distress score drop of ≥1.5 points over 6 months of active use | 18 months | Consented PROMIS + Distress Thermometer trend analysis on aggregate, never individual. | Consented PROs |
| Clinician time-saved | ≥ 20 min saved per survivorship visit using the dashboard vs. chart-diving | 12 months | Optional in-app time-saved survey after case reviews; participation is voluntary. | In-app survey |
| Patient trust | Net Promoter Score ≥ 50 from survivors and ≥ 40 from clinicians | 12 months | Quarterly consented NPS prompt inside the app. | In-app survey |
| Equity of access | No cohort defined by age band, primary cancer, rural/urban, or insurance type has < 70% of overall MAS | 24 months | Optional self-reported demographics with clear opt-out; equity report published annually. | Consented analytics |
How we gather the data — organically, with permission
- Default off. Analytics are disabled until you turn them on. Nothing is inferred, nothing is bought from third parties.
- Event-level, not PHI-level. We store event name, path, tool key, role, and optional numeric values. No free-text notes, no symptoms, no medications.
- Consented PROs. Aggregated PROMIS / Distress / FACT-Cog trends only if you opted in. Individual scores never leave your account.
- Withdrawable anytime. Toggle off and we stop collecting immediately. You can request deletion from the History & Export page.
- Equity by opt-in demographics. Voluntary self-report; used only for equity reporting, never for targeting.
Consent terms are surfaced at signup and on the HIPAA readiness page. Pre-HIPAA, no PHI leaves your device without your explicit export action.
