Cardio-oncology · Case teaching

How the heart is watched during cancer treatment.

Follow one fictional survivor through anthracycline and HER2-targeted therapy, visit by visit, to see how heart biomarkers and imaging are read together.

Synthetic case. All values are simulated. This person does not exist. These are not your results, and this is education, not individual medical advice. Nothing here reads your records or any device.

Baseline

Before anthracycline. Baseline sets the personal reference point.

hs-troponin T6 ng/L
baseline524wk 0wk 12wk 24wk 36wk 52wk 104

A protein released when heart muscle cells are stressed. A rise from your own baseline matters more than one value.

NT-proBNP80 pg/mL
baseline72341wk 0wk 12wk 24wk 36wk 52wk 104

Released when the heart is under strain. Age, kidney function and weight all shift it.

Ejection fraction (LVEF)62 %
baseline5068wk 0wk 12wk 24wk 36wk 52wk 104

Share of blood pumped out with each beat. Measurement varies by a few points between scans.

Global longitudinal strain20.5 % (absolute)
baseline1523wk 0wk 12wk 24wk 36wk 52wk 104

How much the heart muscle shortens. It often changes before ejection fraction does.

Clinical nuance

  • Cardio-oncology guidance (ESC 2022) treats a relative strain drop above 15% or a new troponin or NT-proBNP rise as signs of possible early heart injury, even with a normal ejection fraction.
  • Cutoffs depend on the specific lab test. Troponin I and T are not interchangeable.
  • Ejection fraction can differ by about 5 points between scans, so the same method and lab help.
  • Kidney function, infection, anemia and atrial fibrillation can raise these markers without treatment-related injury.
  • Decisions to pause, continue or protect the heart during treatment are shared between oncology and cardiology teams.

Next steps to talk through

  1. Ask whether you had a baseline echocardiogram and biomarkers before treatment.
  2. Ask which assay your lab uses, since troponin and NT-proBNP cutoffs differ by test.
  3. Ask whether the same imaging method will be used each time so numbers are comparable.
  4. Ask what heart symptoms should prompt a call: new breathlessness, swelling, palpitations or chest pain.
  5. Ask who will follow your heart after treatment ends, and how often.
