Life Spark Survivorship Institute · strategic partner of the CSPN clinician network

Survivorship care that stays with you.

Built for survivors, caregivers, and the clinicians who care for them.

For survivors and caregivers: a private survivorship toolkit to gather the symptoms, long-term effects, and life-transition details that treatment summaries can miss — then generate a transition-of-care plan you can print, save, or hand to your next physician yourself.

For clinicians and care teams: a guideline-aligned survivorship workspace with de-identified cohort trends, cascade and deprescribing worksheets, and FHIR R4 shapes — license it into your existing oncology or primary-care practice.

Two environments, one evidence core

Knowledge for the life that continues through and beyond cancer.

The survivor experience and the professional experience are deliberately separate — their own navigation, language, disclosures, and tools. Choose your door.

For survivors & caregivers

Learn, prepare, and live well after treatment

Trusted education, narrated learning, wellness topics, and tools that help you prepare for conversations with your healthcare team. Designed to work without you entering detailed medical records.

  • Plain-language topics: fatigue, sleep, heart health, cognition, work, nutrition
  • Appointment discussion guide you can print and carry
  • Premium narration so you can listen instead of read

For healthcare professionals

Survivorship evidence and guideline intelligence

A professional environment: evidence summaries, side-by-side society guidance, publication dates, areas of agreement and disagreement, and transparent sourcing you can inspect before you act.

  • Guideline comparison with evidence quality and dates surfaced
  • Cardio-oncology and late-effects reference library
  • Visit templates, shared-care models, and care-plan scaffolds

Educational content for survivors and caregivers is not medical advice and does not diagnose or determine the cause of an individual symptom. Professional content is intended for licensed clinicians exercising independent judgment.

For clinicians & care teams

Built to sit alongside your clinic — not in front of it.

A guideline-aligned survivorship workspace you can license into an existing oncology or primary-care practice. Your treating relationship, your EHR, your orders. We give you the cascade, the surveillance library, the deprescribing worksheet, and the FHIR shapes to make the survivorship visit worth the slot.

Fits your existing EHR

Life Spark is a survivorship toolkit, not a parallel record. Patients hand you a snapshot or PDF — you review it inside your workflow.

Turns visit prep into 5 minutes

Cascade builder, deprescribing worksheet, and cardio-onc surveillance library surface the signals worth acting on before the patient sits down.

Never handles PHI on your behalf

No auto-push. No implicit clinic-owned record. Every clinician view happens through a time-limited, revocable, patient-generated snapshot.

Guideline-cited, not opinion

Every worksheet cites NCCN, ASCO, or COG-LTFU by section — your team can audit provenance without leaving the page.

What's inside

A working survivorship platform, not a brochure.

Symptom & side-effect tracking

Daily severity logs with red-flag detection, exported as clean visit summaries.

Distress & cognition screens

NCCN Distress Thermometer, PHQ-2/GAD-2, and FACT-Cog-style tracking with historical charts.

Late-effect surveillance

Cardio-oncology, bone health, second-cancer, endocrine, and fertility follow-through prompts.

Financial toxicity triage

COST-style screen paired with copay, transport, and lodging program directories.

Clinician trend dashboards

Population-level views with escalation bands so real risk surfaces above noise.

FHIR R4 export + consent

LOINC-coded Observation bundles you download and hand off — nothing is transmitted on your behalf.

Private access audit trail

Every access to your record is logged and visible to you. You own the log and can export it any time.

Hard numbers · program targets

Traction we're building — not a hypothetical.

Quarterly targets for survivors served, CSPN-verified clinicians on the platform, and transition-of-care plans exported. Live actuals are pulled from the platform's own KPI stream — cohort detail lives in the impact dashboard.

Baseline: Q1 2026 · Horizon: Q4 2027

Survivors on platform

30ktarget · Q4 '27

250× vs. Q1 '26 baseline

Q1 '26Q4 '27

PCPs & clinicians (CSPN)

1.1ktarget · Q4 '27

Q1 '26Q4 '27

Transition-plan exports

9.4ktarget · Q4 '27

Targets are projections used for planning, not historical performance. Consented cohort actuals are aggregated in /admin/impact from KPI events (care_plan_completed, transition_plan_exported,clinician_review_logged, etc.).

Data sovereignty

Your record. Your consent. Your export.

The Institute persists your tool logs so trends survive across visits — and hands you a revocable FHIR R4 export you can share with any future clinician. Every clinician view of your data is written to a PHI access log you can read yourself.

Compliance posture

  • Role-based access — patient / clinician / admin, enforced at both the route and the row-level policy.
  • Row-level security on every table containing patient data.
  • Audit trail for every clinician read/export of another user's PHI.
  • Consent-scoped exports — category-level FHIR filtering with reminders.
  • Pre-HIPAA note. Until a BAA is executed and administrative safeguards are complete, use the Institute with de-identified or self-owned data only. See the transition-gap guide for what to route through your health system in the meantime.

Start with one tool. Build the record from there.

Most survivors start with distress or sleep, then add symptoms, cognition, and a transition plan as they need them. Every log feeds the same longitudinal record.

Straight answers

Privacy, ownership, and what happens next.

The six questions survivors and primary-care physicians ask us most, answered without hedging.

  • Yes. Every entry you make is stored in a row scoped to your account with row-level security enforced by the database itself — not by app code you have to trust. Nobody else, including us, reads your record. Full posture on the HIPAA-readiness page.
