Troponin and Natriuretic Peptides in Survivorship: What They Add and What They Miss
Where cardiac biomarkers genuinely change management during and after cardiotoxic therapy, how to interpret a mildly raised value in an asymptomatic survivor, and when imaging remains the answer.
Troponin and natriuretic peptides are cheap, repeatable, and easy to over-interpret. Used with a clear question they add real information to cardiotoxicity surveillance. Used as a routine screen without a plan for the result, they generate anxiety and imaging.
Where they earn their place
During anthracycline or HER2-directed therapy, serial troponin can identify patients at higher risk of subsequent functional decline and support closer imaging or earlier cardioprotective therapy. Natriuretic peptides help discriminate cardiac from non-cardiac breathlessness, which is a frequent and genuinely difficult question in survivorship.
In suspected immune checkpoint inhibitor myocarditis, troponin is central and a raised value demands urgent action rather than a repeat in a week.
Interpreting a mildly raised value
Consider renal impairment, sepsis, pulmonary embolism, arrhythmia, and analytical variation before attributing a modest rise to cardiotoxicity. Trend beats a single value; the same assay and laboratory make the trend interpretable.
In an asymptomatic survivor with a cardiotoxic history, a persistently raised value with no alternative explanation justifies echocardiography with strain and a specialist opinion.
What they miss
Biomarkers do not describe valve disease, pericardial constriction, regional wall motion abnormality, or coronary anatomy, all of which matter after chest radiation. Normal values do not exclude structural late effects, so they cannot replace scheduled imaging in a high-exposure survivor.
Building a usable protocol
Decide the question before ordering: risk stratification during therapy, evaluation of new symptoms, or suspected acute myocarditis. Specify the assay, the interval, and the action for each result band, and document who reviews it. A biomarker with no pre-agreed action is a result nobody owns.
Frequently asked questions
- Should troponin be checked routinely in cancer survivors?
- Not as an untargeted routine screen. It is most useful during active cardiotoxic therapy, when evaluating new symptoms, and when myocarditis is suspected, and each order should have a pre-agreed action.
- What does a mildly raised troponin mean after cancer treatment?
- It may reflect renal impairment, sepsis, pulmonary embolism, arrhythmia, or analytical variation. A persistent rise without an alternative explanation warrants echocardiography with strain and a specialist opinion.
