1. What cumulative anthracycline dose actually raises long-term cardiomyopathy risk?
Risk rises continuously with dose, without a clean threshold. Doxorubicin-equivalent cumulative doses above roughly 250 mg/m² carry meaningfully elevated late-cardiomyopathy risk; above 400 mg/m² the risk is high enough that many centers use a shorter surveillance interval as a default. Working consensus from ASCO, ESC cardio-oncology, and COG-LTFU is a baseline echo at entry to long-term follow-up (typically 1–2 years after therapy completion) and then every 2–5 years, tightened for higher cumulative dose, radiation exposure, symptoms, or pregnancy planning. There is no evidence that stopping surveillance at year 10 is safe in the higher-risk exposure groups.
In practice, the most defensible schedule for a stable survivor with anthracycline exposure ≥ 250 mg/m² is echo every 2 years; below that, every 5 years is usually reasonable in the absence of other exposures.
3. When should global longitudinal strain change management?
GLS declines by ≥ 15% from baseline (or an absolute value worse than roughly −18%) in a patient with preserved LVEF is the sensitivity signal cardio-oncology now acts on. It is the earliest reliable marker of subclinical dysfunction and, in active-therapy patients, triggers cardioprotection (ACE-I/ARB, beta-blocker). In survivorship the same signal warrants a cardio-oncology referral even when the LVEF number reads normal.
4. Do troponin or NT-proBNP have a role in stable survivorship surveillance?
Not as routine screening in asymptomatic long-term survivors — the specificity is poor and the false-positive rate high. They are useful when new symptoms appear (dyspnea, fatigue out of proportion, unexplained edema) as an inexpensive early filter before echo escalation, and in the acute setting when ICI myocarditis is on the differential.

Modifiable-risk co-management. Yes, though the pattern differs from anthracyclines: trastuzumab-related dysfunction is more often reversible, and the majority of events happen during or shortly after therapy. A minority of survivors have persistent or delayed dysfunction; anyone with a documented ejection-fraction drop on therapy deserves an echo at 1 year post-completion and reassessment of interval thereafter. Bimodal: most cases within 6 weeks of initiation, but a real second peak months after the last dose. Cardiac reserve is stressed in the third trimester and peripartum. Any survivor with anthracycline exposure, chest radiation, or documented on-therapy dysfunction deserves a pre-pregnancy echo and a cardio-oncology or maternal-fetal cardiology conversation before conception.
