Dizziness on Standing After Cancer Treatment: Causes Worth Chasing

Lightheadedness on standing is common after treatment and rarely investigated. Dehydration, autonomic damage, medication, anaemia, and rhythm problems each need a different answer.

Feeling lightheaded when you stand up is one of the most commonly reported and least investigated symptoms after cancer treatment. There are several distinct causes and they need different answers.

The measurement that answers most of it

Blood pressure taken after lying for a few minutes, then again at one and three minutes after standing, identifies orthostatic hypotension. It takes five minutes, it can be done in any clinic, and it is frequently skipped. Ask for it by name if dizziness on standing is your problem.

Record the heart rate too. A large rise in heart rate with a maintained blood pressure points somewhere different from a fall in both.

The usual causes

Dehydration and reduced intake are common and correctable. Anaemia after treatment produces exactly this picture and is easily checked. Blood pressure medication, diuretics, some antidepressants, and opioids all contribute, and doses set during treatment are often never revisited afterwards.

Autonomic nerve damage from certain chemotherapies affects the reflexes that maintain blood pressure on standing, and this can persist. Adrenal insufficiency after prolonged steroids or after immunotherapy is less common but important, because it is treatable and easily missed. Rhythm problems, including bradycardia and conduction disease after chest radiation, also present as dizziness.

What to bring to the appointment

A week of home readings taken sitting and then standing, a note of when the dizziness happens and what you were doing, your full medication list including anything started or stopped recently, and your treatment history. That set usually gets to an answer in one visit.

Practical measures while it is being sorted out

Stand up in stages rather than at once, particularly from bed. Keep fluid intake up unless you have been told to restrict it. Avoid standing still for long periods, and be careful with alcohol and hot baths, both of which worsen it. Compression stockings help some people. Ask specifically whether any of your medications can be reduced or retimed.

Fainting with no warning, dizziness with chest pain or palpitations, or dizziness with weakness or speech difficulty needs urgent assessment rather than these measures.

Frequently asked questions

Why do I feel dizzy standing up after cancer treatment?
Common causes include dehydration, anaemia, medications set during treatment and never revisited, autonomic nerve damage from some chemotherapies, adrenal insufficiency, and heart rhythm or conduction problems.
What test checks dizziness on standing?
Blood pressure and heart rate measured lying down and then at one and three minutes after standing. It takes about five minutes and identifies orthostatic hypotension.
