Heart Failure Warning Signs Survivors Should Never Wait Out

Breathlessness, swelling, and sudden weight gain read differently after cardiotoxic cancer treatment. What to watch for, what to log, and when to call the same day.

Most days, breathlessness after cancer treatment is deconditioning, anaemia, or fatigue. Sometimes it is the heart. These are the patterns that should not wait for the next scheduled appointment.

The patterns that matter

New or worsening shortness of breath during activities you managed a month ago. Breathlessness when lying flat, or waking at night short of breath. Swelling of the ankles, legs, or abdomen. Weight gain of two to three pounds in a few days without a change in eating. Persistent light-headedness. A heartbeat that is fast and irregular at rest.

Any of these after anthracyclines, HER2-targeted therapy, or chest radiation should be raised the same week rather than saved for a routine visit.

What to log before you call

Note the date the symptom started, what activity brings it on, how many pillows you sleep on, your daily morning weight for the past week, and your usual blood pressure and pulse. That short record turns a vague call into an actionable one and often decides whether you need imaging.

When it is an emergency

Chest pain or pressure, fainting, severe breathlessness at rest, or a sudden racing irregular heartbeat are emergency symptoms. Call emergency services. Tell them you received cancer treatment that can affect the heart, and name the drugs and radiation if you know them.
