Multidisciplinary Survivorship Workflows Without a Dedicated Clinic

Most practices cannot staff a survivorship clinic. Distributing cardiology, primary care, oncology, pharmacy and behavioural-health roles across existing visits is the realistic alternative.

The literature describes multidisciplinary survivorship care; most practices cannot staff a dedicated survivorship clinic. The workable alternative is distributing defined survivorship roles across visits that already happen, with explicit ownership so nothing depends on a clinic that will never be funded.

Start from the visits you already have

Survivors typically have recurring primary care contacts, and often cardiology, endocrinology or pharmacy contacts as well. Mapping existing touchpoints first shows how much surveillance can be absorbed without new appointment types.

The gap is rarely clinical capability. It is that no one visit type currently owns the survivorship checklist.

Assign roles explicitly

Primary care usually holds the longitudinal record, cardiovascular risk-factor management and coordination. Cardiology or cardio-oncology holds imaging interpretation, cardioprotective therapy and escalation for detected dysfunction.

Oncology holds recurrence surveillance and treatment-history accuracy. Pharmacy holds interaction review and deprescribing. Behavioural health holds distress, fear of recurrence and adherence barriers. Rehabilitation and dietetics hold function and body composition.

Build the fallback before the workflow goes live

The predictable failure is a missed appointment that nobody reorders. Decide in advance who notices an overdue surveillance item and what they do, and record it in the plan itself.

A simple recall list of high-risk survivors with next-due dates catches more than a more sophisticated system that depends on each patient attending.

Use asynchronous coordination rather than meetings

Standing multidisciplinary meetings are the first thing to be cancelled. Shared documentation with named owners, plus a defined escalation path for abnormal results, achieves most of the coordination benefit without a recurring calendar commitment.

Reserve case discussion for genuinely ambiguous situations: discordant imaging, competing risks, or treatment decisions where oncology and cardiology priorities conflict.

Measure two things, not twelve

Track the proportion of identified survivors with a documented exposure summary, and the proportion of high-risk survivors with no overdue surveillance item. Those two numbers expose almost every real failure mode.

Expand measurement only once both are reliably above target, since additional metrics tend to consume the attention that the workflow itself needs.

Frequently asked questions

Can this run without a cardio-oncology service nearby?
Often yes for lower and intermediate risk, with general cardiology holding imaging and cardioprotective therapy and a defined referral or telehealth pathway for complex or discordant cases.
What is the single highest-yield first step?
Identifying the survivor population and capturing treatment exposures in a structured, retrievable field, because every other surveillance decision depends on it.
