The First Survivorship Visit in Primary Care: A 20-Minute Playbook

A structured agenda for the first visit after oncology discharge — exposure reconstruction, cascade set-up, three screens, and what to defer to visit two.

The first visit after oncology discharge tends to expand to fill any amount of time and still miss the structural items. This is a twenty-minute agenda that produces a usable surveillance plan and defers everything that can wait.

Minutes one to six: reconstruct the exposure list

You need diagnosis and stage, systemic agents by class with cumulative dose where it matters, radiation field and approximate dose, surgical history, transplant status, and age at treatment. A treatment summary from the oncology team is ideal; a patient-carried transition plan is the practical substitute.

When cumulative dose is unavailable, record that explicitly rather than leaving the field blank. An unknown anthracycline dose is managed differently from a documented low one, and the distinction should survive into the next clinician's view.

Minutes seven to thirteen: build the cascade and put it on a calendar

Map each exposure to its surveillance obligations: cardiac imaging cadence, lipid and metabolic screening, second-cancer screening beyond the general population schedule, endocrine and thyroid checks, bone density, and fertility or reproductive status.

Convert the cascade into dated orders and recall entries during the visit. A plan documented as a paragraph in the note is a plan that expires with the visit; a plan with dates attached survives staff turnover and patient relocation.

Minutes fourteen to eighteen: three screens

Distress, cognition, and financial burden. Each takes under two minutes, each is under-detected in this population, and each changes what the patient is able to do with the rest of the plan.

Add a fatigue and exercise-tolerance question here. It is the highest-yield single symptom question in survivorship follow-up and it feeds directly into the cardiac branch of the cascade.

Minutes nineteen to twenty: define visit two

Name what is deferred and when it happens: deprescribing review, vaccination catch-up, detailed lifestyle work, and any specialty referral that is not urgent. Deferral with a date is a plan; deferral without one is attrition.

Frequently asked questions

What if the oncology treatment summary never arrives?
Proceed with a patient-reported reconstruction, mark the uncertain fields, and manage to the higher-risk interpretation until documentation arrives. Waiting for records is the most common way surveillance never starts.
How often should the survivorship-specific visit repeat?
Annually for most stable long-term survivors, with the cascade intervals running independently on their own schedule between visits.
Who owns escalation when something abnormal turns up?
Define it in writing at the first visit. A shared care agreement naming who orders, who reviews, and who escalates prevents the abnormal result that everyone assumed someone else saw.
